Some children have medical, learning, or attention issues that can impact school performance.  They may need classroom accommodations, modifications, or other supports in order to participate in the general education curriculum.  These measures remove barriers to learning and are documented in a student’s 504 Plan.

For the child who is already identified as a student with a 504 Plan, the hospital teacher requests a copy of the home school document. Once received, the 504 Plan is reviewed by the teacher to determine if the stated accommodations or curricular modifications are appropriate based on the current hospitalization. Any needed changes to the 504 Plan are made with parent permission and in collaboration with the student’s home school.
